The ALIVE Academy Policies and Procedures

Deposits
A $50 deposit is required to secure your appointment time. The deposit is non-refundable, with 1 complimentary reschedule. The $50 deposit is applied to your appointment fee. The remaining difference is due upon arrival to your appointment.

For Nutrition, Biofeedback, Consultation, Chiropractic, Acupuncture

$50 no show fee

$25 for late cancellation – less than 24 hours

There are no refunds for services and appointments where services have already been rendered.

For services: vibration, sound/light 100% charge for no-show 50% charge for late cancellation

Payment
All payments for services, packages and programs are due up front.  Unless we have an arrangement. Your credit card will remain on file in case of late cancellations or no-shows.

Refunds
There are no refunds for deposit fees and consults. If you cancel after your 1st consult and wish to cancel your 2nd appointment, no refunds will be administered as you have already received services, and the practitioner’s time. For packages and programs there is a 3 day cancellation period. After the 3 day period all sales are final. No refunds will be administered. The $50 deposit fee is non-refundable with 1 complimentary reschedule. If you reschedule more than once another $50 deposit will be required and  $50 NOT $100 will be applied to your appointment balance.

Cancellations
For service cancellations less than 24 hours before your appointment you will be charged 50% of the fee or it will be deducted from your pre-paid package. No-Shows will be charged 100% of the service charge or it will be deducted from your pre-paid package.

Product Sales
All Sales are Final.

Tardiness
If you are late to your appointment this cuts into your treatment time.  A longer appointment time than was originally slotted will not be allowed.

 

The ALIVE Academy and its affiliates reserves the right to refuse service to anyone.
